therapist completes patient assessments, establishes patient care
plans, administers patient treatment and directs delivery of
treatment utilizing professional physical therapy assistants in
accordance with licensure and regulatory scope and standards, and
established hospital policies, procedures and protocols, adhere to
the professional standards of CPH, the American Physical Therapy
Association and NYS Office of Professions. Actively supports and
participates in patient coverage efforts across all departments as
necessary. Hours may vary including weekend rotation for the
inpatient department. STATUS: Full timeLOCATION: Potsdam &
Gouverneur - Travel requiredDEPARTMENT: Physical TherapySCHEDULE:
7.5 hour shiftATTRIBUTES
- Required: active and clear NYS PT license or temporary permit
following NYS Practice Act guidelines.
- Required: BLS (current or scheduled)
- Required: Master's Degree in Physical Therapy as of 8/12
- Preferred: minimum of 1 year of post-licensure practice as a
Physical Therapist RESPONSIBILITIES
- Completes physical therapy evaluations to establish treatment
plans and home programs to restore function, relieve pain & prevent
disability. Adheres to all hospital and departmental policies and
procedures.
- Administers patient treatment plan involving application of
physical therapy techniques and modalities as indicated. Monitors
response and progress to update and modify treatment plan as
necessary to meet patient goals. Instructs patients and family
members / caregivers in physical therapy procedures and self-care
techniques. Directs care that is provided by the physical therapy
assistant by providing oversight. Performs discharge planning and
documentation to provide discharge instructions and maintain
post-discharge continuity of care.
- Consults with physicians and other health care professionals
regarding treatments, progress, patient needs, and outcomes.
- Participates in self and others' education, training and
development, as applicable .Performs any other related duties as
assigned.
- Additional documentation that is not performed during
evaluation and treatment sessions. EDUCATION:LICENSES /
CERTIFICATIONS: PHYSICAL REQUIREMENTS:M - Medium Work - Exerting 20
to 50 pounds of force occasionally, and/or 10 to 25 pounds of force
frequently, and/or greater than negligible up to 10 pounds of force
constantly to move objects; Requires frequent walking, standing or
squatting. For disease specific care programs refer to the program
specific requirements of the department for further specifications
on experience and educational expectations, including continuing
education requirements. Any physical requirements reported by a

be considered for accommodations. PAY RANGE:$39.00 - $52.00 CITY:
Potsdam POSTAL CODE: 13676 The listed base pay range is a good
faith representation of current potential base pay for a successful
full time applicant. It may be modified in the future and eligible
for additional pay components. Pay is determined by factors
including experience, relevant qualifications, specialty, internal
equity, location, and contracts. Rochester Regional Health is an
