Utilizing the nursing process, the centralized prescription refill nurse provides prescription related medication support to patients, including medication refills, prior authorization requests as directed by a provider order via electronic or telephone communication for established primary care patients with Glens Falls Medical Group Primary Care practices.

Responsibilities

Uses the nursing process to collect and document information in the electronic medical record (EMR) from patients requesting prescription refills for all primary care practices.
Provides safe, ethical, direct and indirect patient care that is, patient centered and evidenced based
Participates in inbox management with other members of the care team for all primary care practices.

Qualifications

Education/Accredited Programs

Graduate of an accredited practical nursing program
1 year of recent ambulatory care experience- preferably in primary care

Licenses/Certifications/Registrations

Current unrestricted NYS LPN license

Skills/Abilities

Microsoft Office application experience preferred
Epic EMR experience preferred

Salary Range
The expected base rate for this Glens Falls, New York, United States-based position is $28.00 to $35.06 per hour. Exact rate is determined on a case-by-case basis commensurate with experience level, as well as education and certifications pertaining to each position which may be above the listed job requirements.
