  • Job Requirements
  • American Heart Association certification is required for all direct patient care personnel.
  • Must be dated within 30 days of start. For any position requiring a license or certification, attach scanned on-line verification page with a machine-generated date indication when the verification was performed within past 30 days.
  • All engaged contractors must have page 27 signed form on file in Trinity VMS.
  • Trinity Health Confidentiality of Patient and Proprietary Information and Information System Security Agreement must be on file.
  • Attach copy of specialty exam must be completed within a year.
  • Proof of COVID-19 vaccine, declination or exemption(with agency letterhead). Declination not accepted for CA or IL.
  • Attached scanned proof of 2 MMR vaccinations with dates or primary source IgG titer with lab print-out.
  • Hepatitis B vaccination series required. Proof of vaccination must be uploaded into Trinity VMS.
  • Attached scanned proof of 1 Varicella vaccination with date or primary source IgG titer with lab print-out.
  • Tdap required for *all* contractors.
  • Provide documentation from an exam within the last year that candidate is fit to work.
  • Resume, Skills Checklist within a year, 2 Professional References required for Trinity Profile.
  • All engaged contractors must have signed Trinity Conflict Of Interest Disclosure Form on file.
  • Federal, Nationwide, County Criminal background checks from last 7 years required.
  • Attach completed 3rd Party Screening Company report of candidates highest level of academic education.
  • Attach scanned OIG result print-out or result provided by third party screening company.
  • Must have Trinity NSO check within 30 days of start.
  • Trinity System Access Form must be completed and uploaded one week before start.
  • 2 Step PPD OR IGRA (Quantiferon) is required.
  • Trinity OFAC must be completed within 30 days of start.
